Section 8 – Parent/Guardian Consent Form (To be filled out by a parent between athletic seasons when a physical has already been received by the athlete in a prior season)
Section 9 – Physician Recertification After Injury Form(To be filled out when an athlete is injured and sees a physician; must be submitted before the athlete returns to play; this could be during a season or between seasons)
